Menopause and perimenopause are not always straightforward

Perimenopause is the stage leading up to menopause, when hormone levels begin to fluctuate. Menopause itself is confirmed once 12 months have passed without a period.

Most women seek help during perimenopause, when symptoms can be changeable and harder to interpret. For some, the pattern is familiar: cycle changes, disturbed sleep, flushes or night sweats. For others, it is less obvious: anxiety, poor concentration, joint aches, low mood, changes in weight, or a sense that they no longer feel like themselves.

Hormones may be part of the picture, but they are not always the whole answer. Similar symptoms can also relate to thyroid function, iron levels, stress, sleep or mood. A 30-minute appointment gives your GP time to understand the pattern properly, decide what is worth checking, and help you make sense of what is happening.

What a menopause assessment covers

A menopause appointment is a careful review of what has changed: your cycle, symptoms, medical background, medication, contraception, previous hormone treatment and what you most want help with.

Dr Al-Saraf, or one of our GPs, will usually ask about:

  • Cycle regularity and any change in bleeding pattern.
  • Vasomotor symptoms such as flushing and night sweats.
  • Sleep quality, mood, anxiety and cognitive symptoms such as poor concentration.
  • Vaginal and urinary symptoms, and effects on libido.
  • Current medication, contraception and any previous hormone treatment.
  • Personal and family history relevant to treatment decisions, including cardiovascular and cancer risk factors.

By the end of the appointment, you should understand what may be happening, whether any tests are useful, and which treatment options are appropriate for you, with the reasoning explained clearly.

Are blood tests always needed?

For many women over 45, perimenopause and menopause can be assessed from symptoms and menstrual history.

Testing can be useful when symptoms start earlier, the pattern is unclear, or another cause needs to be considered. Thyroid changes, iron deficiency and other health factors can produce similar symptoms or sit alongside hormonal change.

If blood tests are arranged, they carry an additional fee and are interpreted in context, alongside your symptoms and cycle history. Your GP will go through the results with you.

When HRT may be prescribed

HRT can be an effective treatment for menopausal symptoms, and where it is clinically appropriate your GP can prescribe it and send the prescription to your preferred pharmacy or dispense it in-house. The decision is made after a careful assessment of your symptoms, medical history, stage of menopause and preferences.

Your GP will talk through whether HRT is likely to help, which type and route may suit you, and any risks or side effects that are relevant to your situation. This may include:

  • Which type and route of preparation may suit you – tablets, patches or gels.
  • Whether combined treatment is needed to protect the womb lining.
  • What side effects or bleeding changes to watch for, and when to get in touch.
  • When your first review should take place.

If HRT is not suitable or not wanted, that is a respectable outcome too – non-hormonal treatments, lifestyle measures and symptom-specific options all have a place.

Menopause care means follow-up, not a one-off appointment

Menopause care often works best with review over time. Some women only need one detailed consultation and a clear plan. Others benefit from follow-up to see how treatment is working, adjust a dose or preparation, review bleeding changes or side effects, or go through blood results if tests were arranged.

Because Basuto is a GP practice, that review can sit alongside the rest of your health. Sleep, weight, mood, cardiovascular risk and screening often come up around the same time, and it helps when your GP understands the wider picture as well as the menopause symptoms.
