At Basuto, weight is approached as a health issue, with proper medical assessment and ongoing GP care.
People come to us for different reasons. Some have been trying to lose weight for years. Some are concerned about blood pressure, cholesterol, pre-diabetes, joint pain, sleep, energy or confidence. Others want a clear medical opinion before deciding whether treatment, including medication, is appropriate.
Your GP will start by understanding your health, history, blood results where needed, medication, lifestyle, and any factors that may be making weight harder to manage. From there, you can discuss the options clearly and decide what makes sense for you.

Underlying causes
Weight gain is sometimes a symptom
Weight gain sometimes has a medical driver, which is why assessment comes before advice. Things we look for include:
- Hormonal changes: the menopausal transition and low testosterone can both contribute to changes in weight and body composition.
- Thyroid and metabolic health: central weight gain can reflect underlying metabolic or hormonal issues that need checking.
- Medication: some commonly prescribed medicines contribute to weight gain – a review can identify them.
- Sleep and energy: poor sleep and fatigue increase appetite and reduce activity.
- Emotional wellbeing: stress, low mood and emotional eating patterns play a major role for many people, and our counselling service can help.
- Lifestyle load: disrupted routines, desk work and caring responsibilities gradually shift habits.
These factors matter because they change what your body is responding to, and what kind of support is likely to help.
Your first appointment
What happens at a weight loss consultation
Your first appointment is a medical review of your weight history and health.
Your GP will ask how your weight has changed over time, what you have tried before, what helped, what did not, and what life looks like now: eating patterns, sleep, activity, alcohol, mood, family history and any symptoms that may be relevant. They will also review your current medication, as some medicines can affect weight or influence which treatments are suitable.
Depending on the discussion, your GP may recommend baseline blood tests, such as HbA1c, glucose control, thyroid function, liver and kidney function, and cholesterol. These can help identify underlying factors and give a safe starting point before treatment. Blood tests are charged separately, and your GP will explain any costs before arranging them. For some patients, a broader review through our health checks may be the right next step.
You leave with a clear plan: what we are checking, what can change, what support may help, and when to review progress.
Treatment options
Realistic plans, reviewed and adjusted over time
Treatment is built around your assessment, and it is honest about trade-offs.
For some patients, the plan is structured lifestyle change with medical monitoring: specific changes linked to what the assessment has shown, with follow-up that looks at health markers as well as weight. If an underlying cause is found, that comes first. If emotional health is part of the picture, our counselling service can work alongside the medical plan.
Where clinically appropriate, GP-prescribed weight loss injections, including GLP-1 based treatments, may be considered after assessment. This includes baseline blood tests, discussion of side effects, and structured follow-up. Medication is not suitable for everyone, and it is not the whole plan. Your GP will explain clearly whether it is appropriate for you.
Weight change is usually gradual, uneven and individual, so the aim is a plan that is medically sensible, realistic and properly reviewed.
Ongoing review
Follow-up with a doctor who knows your history
Medical weight management works best with review over time, especially if medication becomes part of your plan. Follow-up lets your GP check how treatment is working, compare blood results, review side effects, and adjust the plan as your health or circumstances change.
At Basuto, you can continue with a doctor who knows what has already been tried, rather than starting again at each appointment. That continuity is often what makes progress feel more manageable.
If you expect regular appointments, membership can make ongoing review easier and more cost-effective. Access membership brings appointments to £80, and Adult membership includes 10 appointments a year. You can also book appointment by appointment if that suits you better.

Blood tests carry separate fees, depending on what is tested – explained and agreed before anything is arranged.
If prescribed, the prescription itself is included in your consultation fee; you pay the pharmacy for the medicine. Ongoing review is at standard consultation rates, or membership from £30 a month for lower per-appointment costs.
Private and NHS care
Using our weight loss clinic alongside NHS care
You do not need to leave your NHS GP to use our weight loss clinic, and anything we find can be shared with your NHS record where useful. Many patients use Basuto for the assessment, time and follow-up they want now, while keeping NHS care for everything it does well.
If specialist input is needed – for example an endocrinology or bariatric opinion – your GP can prepare a clear referral letter for NHS or private care, included in your consultation fee. There is no catchment area: patients come from Fulham, Parsons Green, Chelsea, Putney, Hammersmith, Battersea and across London, and video appointments suit some follow-ups.

How much does a weight loss consultation cost?
£160 for a 30-minute GP consultation, including any prescription and referral letters. Blood tests carry separate fees, which your GP explains before arranging them. There is no registration fee.
Do you prescribe weight loss medication?
Prescribed weight loss medication may form part of a plan where clinically appropriate, after a full assessment including your medical history and baseline blood tests. Not everyone is a candidate, and we will say so honestly.
Will I need blood tests?
Usually, yes, if treatment is being considered. Baseline tests check markers such as thyroid function, HbA1c, liver and kidney function and cholesterol, to look for underlying causes and establish a safe starting point.
Can the first appointment be by video?
Yes, a first conversation can work well by video. If blood tests, examination or a prescribing review are likely to matter, we will recommend coming in – the practice is 3 minutes from Parsons Green.
What happens at the first appointment?
A full GP review of your weight history, previous attempts, eating and sleep patterns, mood, medication and relevant symptoms, followed by an honest discussion of what is likely to be driving your weight and which options fit your situation.
How much weight will I lose?
We will not promise a number – no honest clinic can. Progress depends on the underlying causes, the plan you choose and your own circumstances. What we do promise is proper assessment, realistic targets and regular review.
Is the clinic suitable if I do not want medication?
Yes. Many patients want investigation of underlying causes and a structured, medically monitored plan without medication. Assessment comes first either way, and the plan is yours to choose.
Do I need to register or leave my NHS GP?
No. You can book directly without registering, there is no catchment area, and using Basuto does not affect your NHS registration.
Can weight loss connect with other health concerns?
Yes, and it often should. Weight links to menopause, testosterone, sleep, mood and cardiovascular risk – your GP can connect the assessment with women’s health, men’s health, counselling or a wider health check as appropriate.
